If you would like to request help, you will need to complete a Registration Form and return it along with a signed copy of our terms and conditions of business and our registration fee. (Please ensure that you familiarise yourself with these prior to returning your Registration Form to us.)

Initial payment for your registration must be made by cheque. Please make your cheque payable to Country Cousins. However future invoices may be paid by cheque, direct debit or electronic banking.
Once we have received your completed Registration Form, we will contact you to discuss your requirements in detail and answer any further questions you may have.
